MEMBERSHIP: Membership is defined as a primary holder and spouse, or domestic partner, living in the same household (proof of residency must be presented at time of enrollment). Dependent children of the primary holder, spouse, or domestic partner under the age of 23 must present proof of residency at time of enrollment. Dependents 23 and older must present proof of residence and financial dependence upon the primary account holder, spouse, or domestic partner.

| MEMBERSHIP NUMBER: Each member is issued a membership card. Members must present
their card to the Member Services Desk to enter into LifeStyleRx. |

| GUEST POLICIES: A guest must be accompanied by an adult member (18 years or older). All guests must register, provide a photo ID and pay a guest fee at the Member Service Desk prior to using the Center. Guests 14 and under will pay the prevailing child's guest fee. Guests 15 years and over will pay the prevailing adult guest fee. A guest is also required to complete a guest waiver, a Physical Activity Readiness-Questionnaire (PAR-Q) and an Assumption of Risk if PAR-Q results deem it necessary. Unregistered guests will be asked to leave LifeStyleRx immediately. Expelled or suspended members may not come as the guest of another member. |

| A guest member may use LifeStyleRx no more than six (6) times per year. Members may pay for multiple guest passes at a reduced daily rate. |

| FITZONE: The
LifeStyleRx Youth Fitness Center, Fitzone, is located on the
second floor. This Center is available to children between the
ages of 6 - 15. There is no charge for the use of this facility
for children between the ages of 6 - 15 if they are listed as
an additional Member on the Membership Application and Agreement.
Children between the ages of 6 - 15 not listed as additional
Members on the Membership Application and Agreement may use
Fitzone by paying the prevailing guest fee. Children not listed,
as additional members on the Membership Application and Agreement
may not use LifeStyleRx and Fitzone more than 6 times annually. |

| This 6-visit policy may be waived from time to
time due to structured youth programming. Youth Fitness Center

| CHILDCARE: The LifeStyleRx
Childcare Center is located on the first floor. This Center
is available to children between the ages of 2 months and 6
years of age. There is no charge for the use of this facility for
children between the ages of 2 months and 6 years of age if they
are listed as an additional Member on the Membership Application
and Agreement. Children between the ages of 2 months and 6 years of age not listed as an additional Member on the Membership Application
and Agreement, may use childcare by paying the prevailing childcare
hourly rate. A limit of two hours per day is allowed for childcare
services. The Childcare Center hours are: |

| EMERGENCY PROCEDURES: Should an accident occur at LifeStyleRx, immediately report
it to the Member Services Desk by using one of the telephones
located throughout the facility or notify any staff member.
Emergency procedures will be expedited immediately. Should you
or a family member become injured while at LifeStyleRx, Center
Staff is not permitted to provide off site transportation. We
reserve the right to call an ambulance. All Members are encouraged
to be CPR certified. In the event of a fire alarm, please do
not evacuate until notified by a staff member. |

| LOST AND FOUND: LifeStyleRx is not responsible for items lost, stolen or damaged.
Lost and found item inquiries should be made through the Member
Services Desk (Security). Items turned in and not claimed after
thirty (30) days will be donated to local charities. |

| TELEPHONE MESSAGES: If you wish to receive a message while at LifeStyleRx, check
in at the Member Services Desk where messages will be held for
you. Paging in the Center is used for emergencies only. |

| FITNESS EVALUATION: All LifeStyleRx Members are eligible to receive a fitness evaluation.
This value added service allows the LifeStyleRx staff to evaluate
a number of physiological parameters including health risk factors.
This information is then used to design an individual exercise
prescription accordingly. If for some personal reason a member
feels that physical assessment is inappropriate, this process
can be waived, however a release to exercise from their Primary
Care Physician or a signed Assumption of Risk form will be required.
The fitness staff may request a written Physician's Consent
pending certain medical conditions. |

| PERSONAL TRAINING: LifeStyleRx Members may only utilize personal trainers employed
by LifeStyleRx for any type of personal training including swimming,
pilates, spinning, yoga and weight and cardiovascular training
to name a few.

| CAFÉ: The Café is a health-oriented restaurant located in the
lobby of the LifeStyleRx Center. The Café is available
to Members, guests and the general public. The menu is designed
to support health oriented, lifestyle changes which coincides
with the total wellness concept being encouraged through LifeStyleRx
membership. Tasty entrees that are reasonably priced and low
in calories, sodium, cholesterol and saturated fat will be available
making the nutritional component of the lifestyle transition
convenient and enjoyable. |

| DAY LOCKERS: Membership is asked to respect LifeStyleRx's willingness to
provide a daily locker services. Rental lockers are available
on a yearly basis. Please do not bring valuables into the center
(or leave then in your locker). LifeStyleRx is not responsible
for items lost, stolen, or damaged. If necessary, valuables
can be secured at the Member Services Desk. |

| TOWELS: Membership
is asked to respect LifeStyleRx's willingness to provide towels.
The Center considers this service a privilege of LifeStyleRx's
membership. If privilege by a few becomes excessive this liberal
privilege may be revoked. Towels are the sole property of LifeStyleRx. |

Exercise Equipment Areas

No Members under age 13 shall be allowed in the exercise equipment area. |

Members 13-14 years of age must complete the Teen Certification Program prior to utilizing the exercise equipment area. |

All teens between the ages of 13-14 must carry their teen certification card. |

An adult must accompany teen members between the ages of 13-14 while utilizing equipment. |
|
Please limit your workout on cardiovascular equipment to 30 minutes during peak hours. |

Instruction in the proper use of all equipment is the responsibility of each member.· |

Therapy Pool
Designed specifically for our Physical Therapy Program. LifeStyleRx
Members may use the therapy pool under the following limited
conditions:

LifeStyleRx organized-supervised (guarded) programs. |

Independent exercise program with physician referral form and adhering to warm water exercise pool policy times. |

The pool is open during center hours only. |

The pool is a multi-purpose facility which is used for lessons, water exercise classes, team workouts and meets, and free swim to name a few. Pool scheduling is subject to change and often includes multiple activities at any one time. |

No food or drink allowed in area. |

Gymnasium
The gymnasium is a multi-purpose facility, which is used for
a variety of scheduled and free-play activities. Unless specifically
reserved, all members will be allowed access with the limiting
factor being facilities capacity.

|
Due to special programming the gymnasium may be reserved with notice from time to time. Advance notice will be posted. |

Group Exercise Studio

Members 15 or older may participate in regularly scheduled group exercise classes. Members 13-14 may participate in classes designated on the schedule as "teen" classes. |

Members and guests are required to wait outside the studios until the next class begins. |

Proper etiquette shall be enforced at all times. |
|
Group Exercise A/V equipment is for the exclusive use of LifeStyleRx staff. |

Class participants are required to sign in prior to each class. |

Class participants are not permitted to enter a class after the initial 10-minute warm-up period. |

Track direction will change every other day. |

Walker and slower runners stay to the inside lane. |

Fast runners stay to the outside lane. |

Runners yield to walkers. |

Sauna, Steam and Whirlpool Facilities
|
For your own safety, please follow posted guidelines at respective areas. Members and guests use these facilities at their own risk. |

Members 12 and under will not be allowed in the sauna, steam and whirlpool facilities. |
|
An adult must accompany a teen Member or guest between 13-14 years of age. |
|

Dress Code
|
Shirts, shorts, sweat suits, leotards, workout tights and athletic shoes must be worn in all areas of the LifeStyleRx Center. Members wearing bathing suits must have proper "cover-up" when outside the pool area. |
