ValleyCare Health System is committed to serving the needs of older adults, their caregivers and their families. Serving both healthy adults and those who are homebound, our services include information, referrals, medical and preventive care.


Bone Density Testing
Bone Density Testing ImageOne out of every two women past menopause has osteoporosis. 75% of women who have osteoporosis don’t even know they have it. It is a potentially crippling disease. Women are not the only ones who develop osteoporosis. According to the National Osteoporosis Foundation, 2 million American men have osteoporosis, and another 12 million men are at risk.

Bone density measurement is a diagnostic test used to measure the amount of mineral in bones. ValleyCare has the latest technology in bone density screening and uses dual energy x-ray absortiometry (DEXA). It involves lying on a table for 10-20 minutes while low dose x-ray beam scans the spine, hip or both. The test is painless and there is no known risk other than minimal radiation exposure.

Medicare has covered bone density tests since 1998 for qualified individuals at risk for osteoporosis. Reimbursement through private carriers is variable. In either case, physician referral is required. Many women find it convenient to schedule this bi-annual screening procedure with their mammogram. Physician referral required. Coumadin Clinic
Coumadin, a blood thinning medication, requires vigilant monitoring because the potential for food or drug interactions and side effects is high. Providing comforting one-on-one support, ValleyCare Clinical Pharmacy staff works with patients to monitor blood tests, ensure adherence and proper dosage, and check for possible complications. Discharge Planning
Discharge Planning ImageValleyCare's case managers and social workers provide professional care and discharge planning to help ease the transition from hospital to home. They provide a wealth of resource information ranging from medical home care and medical equipment to financial support and meals. For more information, call (925) 416-3653.




Flu Shot Immunization
Flu Shot Immunization ImageEvery fall season, seniors can receive a flu shot at our Occupational Health offices located in the Medical Plaza, 1133 E. Health Insurance Counseling & Advocacy Program (HICAP)
Sponsored by the California Department of Aging, assistance is provided for Medicare and other senior insurance issues. HICAP volunteer counselors are available to help compare health plans in an unbiased manner. They also have information on Medicare benefits, supplemental insurance and other health care options. Senior Behavioral Health: A Special Program for Seniors
Senior Behavioral Health Image Legends at ValleyCare Health System, is a short-term, inpatient, voluntary admission program specifically designed for older adults with unique emotional and mental health disorders such as severe memory loss, disorientation, hallucinations, depression and anxiety. A Legends professional is available to provide a free, confidential evaluation in the home or at the facility. Medicare covers hospital services. Meals on Wheels for Seniors
Approximately 450 home bound seniors throughout the Tri-Valley enjoy nutritious, low-cost meals delivered to their homes daily. Beyond helping seniors maintain their quality of life, Meals on Wheels has helped save lives by providing quality food daily.

Pharmacy Clinic / Prescription Advice

Pulmonary Rehabilitation
ValleyCare Health System's innovative Pulmonary Rehabilitation program is committed to helping those with various respiratory ailments lead a full and satisfying life at work, home and in the community. This six-week program consists of exercise, education and support for people with documented lung disease. Physician referral required.

Skilled Nursing Facility
Located at Valley Memorial Center in Livermore, the Skilled Nursing Unit is where patients receive excellent medical care including physical and occupational therapy as they recover from illness or injury before returning home. Senior Housing Complex
In collaboration with the City of Livermore and ValleyCare Health System, Heritage Estates senior living opened in 2004, with assisted living apartments and independent living units. The complex is located across the street from ValleyCare’s Livermore campus on E. Stanley Blvd. Leisure Care Corporation manage services.
